More InformationPathway Advocate, Health Services: work with HIV positive individuals and those who do not know their status, who are incarcerated at state correctional facilities, and assist those returning to Onondaga County, in order to motivate and ensure engagement and adherence in HIV-related treatment and related services. Travel between New York State Correctional facilities (Marcy, Mid-State and Mohawk) to provide universal HIV test offers to incarcerated individuals nearing release; seek out and enroll HIV+ incarcerated individuals not already receiving treatment; provide aggressive follow up services to those released to Onondaga County to ensure treatment adherence, and collaborate with community and corrections partners. The Positive Pathways program works in conjunction with CCA’s CHOICES program which provides additional HIV-related serves in the same correctional facilities. Qualifications: BA/BS in health, counseling, education preferred with knowledge and experience in the HIV/AIDS field, working within the criminal justice system, and motivational interviewing. Training in ARTAS is a plus, or will be provided. Must have reliable transportation and the ability to gain clearance to correctional facilities.
